Reimbursement: Patient onboarding, education and setting up the equipment (billed first month only)
CPT code 99454: 
"Instruments with daily recordings or programmed alerts transmission, monthly." 
Reimbursement: Patients with a instrument for a 30-day period (billed monthly)
CPT code 99457:
                            "Remote monitoring with treatment management services, at least 20 minutes or more of physician/ clinical staff/other qualified healthcare professional time requiring interactive communication with the patient/caregiver during the month."
                            This is different than CCM (chronic care management). 
Reimbursement: Atleast 20 minutes of treatment management required (billed monthly).
The service must be ordered by a healthcare provider
