Monthly device supply for patients with at least 16 days of recordings in a 30-day period.
Important:
Cannot be billed with 99445 in same month.
Device Supply - Short
2-15 DAYS
Monthly device supply for patients with 2 to 15 days of recordings in a 30-day period.
Important:
Cannot be billed with 99454 in same month.
Treatment Management
Treatment - 10-19 Min
SHORT DURATION
Cumulative 10-19 minutes of RPM management per calendar month.
Requirements:
- Requires interactive communication
- Cannot be billed with 99457 same month
Treatment - First 20 Min
STANDARD DURATION
First 20 minutes of RPM treatment management per month.
Requirements:
- Requires interactive communication
- Cannot be billed with 99470 same month
Treatment - Add-on 20 Min
ADD-ON CODE
Add-on code for each additional 20-minute block (40min, 60min, 80min).
Important:
- Can only be used with 99457, not 99470
- Max 3 units per month
Qualifications
- The service must be ordered by a healthcare provider
- Patient must sign an informed consent at the time of onboarding
- Device must be supplied to the patient for at least 16 days prior to a monthly billing period
- Data must be wirelessly synced with the provider dashboard for patient monitoring
- Physician, or a qualified healthcare professional or by clinical staff must monitor the RPM data
- Watch for new rules
Patient Responsibility
- Patient takes daily measurements only
- No need to pair instruments to smartphones or tablets
- Instruments are connected with cellular network
- Vitals are automatically uploaded to our server without any extra effort
